River City’s mission is to provide highly effective programs for individuals suffering from a mental health or substance use disorder, and co-occurring conditions. Using an integrative approach, River City engages each patient, their families, and a variety of treatment providers to develop an individualized and comprehensive Plan of Care.

 

River City has established itself as the premier outpatient solution for those struggling to live a better life because of mental health and substance use issues. River City has developed many relationships with recovery centers, treatment providers, and other providers to help treat each individual on a combined front. Our professional clinicians utilize an array of evidenced-based strategies and curriculums that are as diverse as the individuals we serve. They include, but are not limited to: A New Direction, Moving On, Living in Balance, Illness Management and Recovery, The Matrix Model for Criminal Justice Settings, and A New Freedom. Treatment strategies and competencies include: Cognitive Behavior Therapy (CBT), Dialectical Behavior Therapy (DBT), mindfulness therapies, Ego Psychology, and other strategies that address addictions, mental health, trauma, stress, anger, and a plethora of psychosocial conditions.

 

One of the most important pieces of the addiction recovery program at River City is the development of a solid continuing care and relapse prevention plan. It took years of substance use to develop the addictive behaviors that are addressed in treatment. One or two months of inpatient care is just the first step to recovery. We recognize that continuing care is key to maintaining sobriety, which is why we begin continuing care and relapse prevention planning on day one and continue developing that plan until discharge.

 

Leaving treatment and going back to daily life surrounded by triggers and everyday stressors without a comprehensive continuing care plan is a recipe for disaster. Our counselors at River City work with each client to ensure they have the resources and tools to maintain long-term recovery. Continuing care plans are unique to each individual based on their goals and needs. Our aftercare planning includes making sure clients have access to proper medical care, psychiatric care, substance use counseling, employment, education, and housing.

 

River City can offer a structured outpatient program for your clients or patients on an individual or group basis to fit their schedule.  

 

Program of Care

When recovering individuals are admitted into River City’s programs and services, they will receive the following services:

 

  • Clinical Assessment: We perform a comprehensive bio-psychosocial assessment with each recovering individual, so that we can evaluate their current functioning and needs. We utilize many assessment tools such as the SASSI, PTSD Screening tool, etc. Many of our Clinicians are experienced and certified in ASAM and other treatment protocols.

  • Psychiatric Evaluation:  We have strong relationships and contracts with many psychiatrists, addictionologists, nurse practitioners, and many other allied health professionals. They collaborate with us to provide recovering individuals with a full psychiatric and medication evaluation.

  • Patient-Centered Individualized Treatment Plan: This plan takes into account the recovering individual’s medical, psychiatric, and psychosocial needs. Our Clinicians identify each recovering individual's treatment needs and designs a treatment plan that takes into account the whole person. This includes making referrals to the appropriate community professionals, providing ongoing monitoring of the treatment plan elements, and supporting the recovering individual throughout the lifecycle of the treatment plan.

  • Counseling: Our Clinicians provide intensive counseling and therapy for the recovering individual and/or their family depending on their needs. Depending on a recovering individuals payment method and the assessed level of care,  we can provide counseling in the recovering individual's home, school, place of employment, in our comfortable offices, and in their community. The goal is to develop improved cognitive, emotional, and behavioral functioning that can improve each recovering individual's quality of life.

  • Care Coordination (Case Management): We assist each recovering individual and engage other professionals in identifying, implementing, monitoring, and evaluating other resources to be included in the overall care of the recovering individual. As the lead agency, River City ensures that the recovering individual receives the right treatment at the right time with the right professional. This includes identifying recreational and leisure activities that promote growth and wellness for the recovering individual.
