Resources for the Medical Community

River City Integrative Counseling’s commitment to supporting and collaborating with medical professionals, hospitals, and other allied health professionals is unparalleled. We know that you work tirelessly to improve the health and lives of everyone that you treat. River City also knows that a primary goal your profession and an expected outcome of your work is to:

  • Reduce hospital readmissions

  • Reduce emergency room over-utilization

  • Improve the health and lives of your patients

For years, hospitals have been penalized by multiple commercial payors for patients that over-utilize emergency rooms and when patients are readmitted due to either the same or a different medical condition. River City can help reduce hospital readmissions, reduce emergency room overutilization, and help to improve the health and lives of your patients.

As one of the leading integrated behavioral healthcare organizations in Central Virginia, it is our mission to:

  • Reduce out-of-home placements, such as hospitalizations, treatment and incarceration

  • Reduce emergency room over-utilization

  • Improve quality of life of the patients that we serve

For Physicians, Nurse Practitioners, Physicians, and Prescribing Professionals

As a physician or lead medical professional, you are regularly required to provide comprehensive diagnoses, treatment, prescribe medications, and follow-up medical care for a large number of patients, sometimes at multiple sites. Additionally, your patients present with complex medical conditions and they often experience moderate to severe psychiatric and substance use conditions that require your utmost attention. You are the professional that is ultimately responsible for ensuring the health, safety and welfare for each patient.

River City can help ensure that the hard work that you and your team have provided for patients in your care is not short lived due to them being discharged with no real discharge plan. With just a telephone call, River City will meet with the patient and your discharge team at your location to begin the “soft hand-off” that is often overlooked due to the immediate pressures and demands of a physician and their medical team. While we would prefer to meet with you and the patient days before their discharge, River City is experienced with meeting the needs of busy medical professionals.

Program of Care

We have experience in ensuring seamless transitions for your patients as they integrate back into their communities of choice. Our structured program of care includes, but is not limited to:

  • Clinical Assessment: We perform a comprehensive bio-psychosocial assessment with each patient, so that we can evaluate their current functioning and needs. We utilize many assessment tools such as the SASSI, PTSD Screening tool, etc. Many of our Clinicians are experienced and certified in ASAM and other treatment protocols.

  • Psychiatric Evaluation:  We have strong relationships and contracts with many psychiatrists, addictionologists, nurse practitioners, and many other allied health professionals. They collaborate with us to provide patients with a full psychiatric and medication evaluation.

  • Patient-Centered Individualized Treatment Plan: This plan takes into account the patient’s medical, psychiatric, and psychosocial needs. Our Clinicians identify each patient's treatment needs and designs a treatment plan that takes into account the whole person. This includes making referrals to the appropriate community professionals, providing ongoing monitoring of the treatment plan elements, and supporting the patient throughout the lifecycle of the treatment plan.

  • Counseling: Our Clinicians provide intensive counseling and therapy for the patient and/or their family depending on their needs. Depending on a patient's payment method and the assessed level of care,  we can provide counseling in the patient’s home, school, place of employment, in our comfortable offices, and in their community. The goal is to develop improved cognitive, emotional, and behavioral functioning that can improve each patient's quality of life.

  • Care Coordination (Case Management): We assist each patient and engage other professionals in identifying, implementing, monitoring, and evaluating other resources to be included in the overall care of the patient. As the lead agency, River City ensures that the patient receives the right treatment at the right time with the right professional. This includes identifying recreational and leisure activities that promote growth and wellness for the patient.

With just a telephone call, River City can help you and your loved ones reduce and/or eliminate troubling thinking and behaviors that have caused problems. We can help you obtain the knowledge and skills to function more independently and improve your quality of life.
